addition, a medical student research grant is also awarded to local
students taking part in summer research programs. Over the last 5
years, the Society has awarded approximately $32,000.00 to medical

Annual Clinical Update
|
|
The Omni William Penn Hotel in Pittsburgh
was the site of the March 2010 Clinical Update in Geriatric
Medicine
conference. This annual conference is jointly sponsored by the
Pennsylvania Geriatrics Society – Western Division; the University of
Pittsburgh Institute on Aging; and the University of Pittsburgh School
of Medicine Center for Continuing Education in the Health Sciences.
Previously awarded the American Geriatrics Society Achievement Award
for Excellence in a CME program, this conference continues to grow and
draws prominent international and national lecturers, as well as
nationally renowned local faculty.
Under the leadership of course directors
Drs. Shuja Hassan, Judith S.
Black, and Neil M. Resnick, the program continues to be an extremely
popular and respected resource in the region. Over 350 geriatrics
professionals from all disciplines, including physicians, nurses,
pharmacists, physician assistants, social workers, long term care and
managed care providers, and health care administrators participated in
key plenary sessions, as well as newly featured "Ask the
Physician"
sessions. During these sessions, participants had the opportunity to
pose challenging questions from their own practices to
geriatric-focused subspecialists in the fields of nephrology,
cardiology and neurology.
Program highlights of the three-day
conference included 'Tomorrow's Technology for Today's Care,'
which focused on the potential role of newly available technology to
advance the patient-centered medical home, telemedicine, and the
implementation of an electronic medical record in daily practice. This
well received session was held in conjunction with the Technology for
Life and Living Conference on Friday, March 26. In addition, 'Meeting
In-Patient Criteria'
explored the byzantine world of Admissions vs. Observation and answered
questions as to why is matters to health care providers and their
patients.
Guest speaker Joel Weintraub, M.Ed., B.S.,
physiologist and
humorous educator from Lafayette Hills, PA provided attendees with an
enthusiastic, enlightening and dynamic presentation, "Humor
for the Health of It". Dr. Weintraub made a lasting
impression on participants with his excellent comedic delivery of the
material.
The interactive symposium provided
participants with the opportunity to
experience firsthand the possibilities of an interconnected health care
system. Distinguished speaker Paul Grundy, MD, IBM's Director of
Healthcare Technology and Strategic Initiatives lead the demonstration
using interactive patient stories.
Planning is underway for the 2011 Clinical
Update. Members of
the Pennsylvania Geriatrics Society – Western Division receive a
discount when registering for the conference! To inquire about becoming
a member, contact Nadine Popovich at 412-321-5035 ext 110 or email npopovich@acms.org.

Pennsylvania
Geriatrics Society welcomes
American Geriatrics Society President
A record number of registrants attended the fall program of the
Pennsylvania Geriatrics Society – Western Division to hear guest
speaker, and current American Geriatrics Society President, Cheryl
Phillips, MD, CMD, AGSF. Dr. Phillips presented, '''Practical Approach
to Managing Care Transitions: Skilled Nursing Facility/Emergency
Department.'

Pennsylvania
Geriatrics Society President Michael Yao, MD (left), with American
Geriatrics Society President Cheryl Phillips, MD, CMD, Board of
Director and COSAR representative Neil Resnick, MD and
Secretary-Treasurer Judith Black, MD during the annual Fall program
held at the Monterey Bay.
|
Dr. Phillips presented
details
that poor transitional care from the skilled nursing facility (SNF) to
the Emergency Room (ED) back to the SNF is 'too much of not a good
thing.' Of all the transitions, least is known about SNF-ED-SFN, due to
little documentation about frequency, reason or outcome. It is
recognized as a frequent occurrence, but often not linked with hospital
admission.
It is
estimated that up to 25% of SNF residents
are transferred to ED yearly. Evidence has shown that poor transitions
increase the risk of re-hospitalization and medication errors. She
detailed a number of recommendations for improving care, including best
care practice initiatives, which include communication and engagement;
creating guidelines for a process to allow the desired outcome; define
a measurement and feedback cycle and provide the tools (forms,
procedures, policies) to make it all happen, as forms by themselves do
not change the process.
Dr. Phillips
is the chief medical officer of On
Lok, Lifeways, the parent to the PACE (Program of All-inclusive Care
for the Elderly) model that serves nursing home eligible seniors in the
greater San Francisco bay area. Her credentials include being past
president of the American Medical Directors Association, as well as
serving on multiple national boards and advisory groups for chronic
care. In addition, Dr. Phillips served on the California Commission on
Aging, been a delegate to the White House Conference on Aging and spent
time as a Primary Care Public Policy Fellow at the U.S. Department of
Health and Human Services.
The annual
fall business meeting is
complimentary to members of the Society! For membership information,
please contact Nadine Popovich, administrator at 412-321-5035 ext 110
or email to npopovich@acms.org.
