There is no fee to attend.

There is a cost of $10 for social workers wishing to earn SW continuing education credits, payable the day of the training program. Those seeking such credit should bring cash or a check in that amount payable to Jewish Healthcare Foundation.

Registration Deadline: September 20, 2017

A 2.5 hours online prerequisite course must be completed prior to the in-person program. The link, user name and pertinent information will be provided to registrants approximately two weeks prior to the in-person course date.

Course Description

The major focus is to advance participants’ understanding of advance care planning and the role of POLST and the development of skills to facilitate goals of care discussion for individuals with advanced illness, serious health conditions, medical frailty or those of advanced age and wishing to define their preferences for care.

Course Goals

Participants will
• Be able to define the role of Advance Directives and POLST in Advance Care Planning;
• Have sufficient knowledge of tools and resources to serve as a POLST resource within their institutions and communities;
• Have a high level of understanding of the importance of the POLST conversation;
• Demonstrate the ability to facilitate a POLST discussion and appropriately complete a POLST form.

Who Should Attend

Health care professionals, including physicians, physician assistants, CRNPs, nurses, social workers, chaplains and
others, who care for individuals approaching the end of life and wish to enhance their ability to engage in
effective goals of care conversations with patients, families and surrogates.
CME Accreditation and Designation Statement

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of University of Pittsburgh School of Medicine and the Coalition for Quality at the End of Life (CQEL). The University of Pittsburgh School of Medicine is accredited by the ACCME to provide continuing medical education for physicians.
The University of Pittsburgh School of Medicine designates this live activity for a maximum of 5.5 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Other health care professionals are awarded .5 continuing education units (CEU’s) which are equal to 5.5 contact hours.
NOTE: An additional 2.5 CME credit is earned through the completion of the prerequisite course.
Nurses: These hours may be considered eligible for completing the 30 hours of continuing education required for
biannual nursing re-licensure in Pennsylvania.

CEU Credits for Social Workers

This program is offered for 8.0 hours of social work continuing education through co-sponsorship of the University of Pittsburgh’s School of Social Work, a Council on Social Work Education-accredited school and, therefore, a PA pre-approved provider of social continuing education. 2.5 of the 8.0 CEU credits are earned through the completion of the prerequisite course. These credit hours satisfy requirements for LSW/LCSW, LPC
